HPH Hospice is a non-profit organization that offers a wide variety of post-acute care services including palliative care, home health care, continuous care, hospice care and bereavement services. HPH Hospice never turns anyone away that can not make payment. At HPH Hospice patients and families come first.
Chapters Health Foundation fundraises to provide Charity Care to residents in Pasco, Hernando and Citrus Counties for patients who are uninsured or underinsured. All monies donated stay in the county in which they were raised. The foundation also provides the funding for our bereavement services for adults and children. Individual and group counseling are available at no charge to all residents in the counties we serve. The foundation also provides a weekend camp each year for grieving children ages 6-17, at no charge.
If you would like to sponsor or donate to the Charity Care or bereavement services, please contact Kelly Steen at 727-819-5923 or SteenK@ChaptersHealth.org
